Blizzard Entertainment has reviewed player feedback on Diablo IV and announced several changes affecting Mythic and Iconic Mythic items. Developers aim to simplify the system and give players more control when crafting an item that should "elevate a character to the pinnacle of power."

The methods for creating Mythic unique items remain the same, but the outcome is now much more predetermined. Players will be able to craft such items in two ways – directly upgrading an item to Mythic using the Horadric Cube or crafting an item at the Jeweler.

Diablo IV developers stated that directly upgrading an item to Mythic in the Horadric Cube is a more expensive method compared to upgrading at the Blacksmith or Jeweler, but it guarantees that the item will not lose its properties – no random factors. Only one restriction will be set: players will not be able to use this method with unmodifiable items.

Mythic unique items directly upgraded in the Horadric Cube will receive a "crafted item mark." Only one item with such a mark can be used at a time.

The "Rune Crafting" recipe at the Jeweler will allow players to create Mythic unique items for their class. This requires Dazzling Sparks and Runes. By selecting the desired slot, for example, a one-handed weapon, the player will receive a corresponding random Mythic item for the desired class. Items crafted by the Jeweler will not have the aforementioned mark.

The Blacksmith's Mythic Cache will create Mythic unique or Iconic Mythic items for the player's class. Such items will also not have the crafted item mark.
