Modder RPCockien recently returned to The Witcher 3: Wild Hunt and introduced a modification that updates Geralt's image.

He made dynamic cat eyes for the witcher. The reflection of light depends on the environment — the eyes can burn like candles, but do not emit light themselves.

So far, the mod is only available for Geralt: depending on the situation, the protagonist's eyes will change.
