The main character will be Makoto Daito, a Japanese-American who goes from a stowaway refugee to a legend of the criminal underworld. The gameplay will offer an unusual and brutal combat system where the left and right sides of the character's body are controlled separately. This will allow for manual combinations of blocks, jabs, grabs, and cold weapon attacks.
Another important part of the gameplay will be the music production mechanic: the hero will be able to scout for talent on the streets and record tracks, collecting samples from ambient sounds. A real sensation was the appearance of rapper Tupac Shakur. His likeness was officially licensed with the support of the musician's family and Snoop Dogg, who also plays one of the main roles.

The expansion's story will unfold in the winter of 1905 in Sicily and will neatly fit into the one-year time jump of the main story, immediately after the protagonist Enzo Favara becomes a full-fledged "soldier" in the Torrisi family. Man of Honor will add two new story chapters in which Enzo crosses paths with the young, daring, and ambitious Ennio Salieri, the future don from the first Mafia. During this period, he is planning to move to America.
Salieri's appearance will cause a rift in Enzo's relationship with his close friend Cesare Massaro. The developers completely rejuvenated the character's appearance based on actor Glenn Taranto's likeness, and Andrew Russell is now responsible for the voice acting and motion capture of young Salieri.

The Judge controls traps from the shadows, uses hidden corridors and hallucinogenic gas, and can summon an Accomplice to capture victims. However, he is physically vulnerable and cannot win in open combat.
Three other players take on the role of the Accused. Their goal is to cooperate, collect keys, and find a way out of the maze before the timer runs out. A unique feature will be "rehabilitation traps": once caught, the player must choose whether to sacrifice a body part, which will permanently slow them down or complicate puzzle-solving, or wait for teammates' help.
